Disability personal assistance services
This toolkit has information to help youth with disabilities transition into adulthood. It explains the different kinds of personal assistance services (PAS) that can help a young person become independent. Supports may include getting help with homework, learning to drive, help with getting dressed, work supports, or being cued to complete an activity. The guide is organized so that youth only use what they need, choose from examples that match their own situations, and decide on strategies they would like to try. This toolkit can help transition-age youth with significant disabilities and their family and friends navigate the complex world of PAS to help them make the best choices to become self-sufficient.
 
Disability self advocacy
This video explains to young people with medical conditions or disabilities the importance of taking care of their health as they transition into adulthood and take responsibility for themselves. Includes information on taking medicines, talking with doctors, carrying an emergency health information card, keeping a health care notebook, paying for health care, going to college and planning for accommodations, eating the right foods, exercising and more.
